As Transplant Program Assistant, you will coordinate administrative support for the Transplant Program across all phases of care, from referral for evaluation to post-transplant follow-up. However, this role is so much more than simple scheduling or completing evaluations – you are integral to the success of the Transplant Program, ensuring patients have access to the life-saving treatment they need. This department is very busy, so you have an additional opportunity to work in a fast-paced environment that stretches your skills and enhances your ability to provide exceptional customer support. 

 

A caregiver in this position works days from 8:00AM – 5:00PM. 

 

A caregiver who excels in this role will:  

  • Coordinate referrals for transplant evaluation. 

  • Perform initial patient intake interview, register patient in EMR, submit for financial clearance, obtain outside records for review, create a work file for the coordinators, schedule all evaluation appointments and scan records into EMR. 

  • Develop Selection Committee meeting agendas and document meeting minutes. 

  • Ensure administrative support is provided to various transplant team members. 

  • Send correspondence to referring physicians at the time of evaluation, transplant and discharge. 

  • Schedule and communicate follow up appointments for transplant patients. 

  • Triage phone calls from patients and direct to appropriate clinical team member. 

  • Provide MELD score updates for UNOS. 

  • Ensure patients schedule labs, fax orders to outside labs and obtain results. 

  • Enter results into Transplant Database and copy records for EPIC scanning. 

  • Assist with phone refill requests. 

  • Serve as liaison to Case Managers. 

 

Minimum qualifications for the ideal future caregiver include:  

  • High School Diploma or GED 

  • Three years of experience in a hospital, medical or other administrative setting, including appointment scheduling, public contact and a role coordinating activities of medical and administrative personnel 

  • Knowledge of medical terminology 

 

Preferred qualifications for the ideal future caregiver include:  

  • Surgical scheduling or appointment setting experience 

  • Transplant experience 

  • Knowledge of computers, software programs and data entry 

  • Associate's Degree or additional education from accredited program
